Fatal machine had caused injury before
The machine at Cemex which exploded and caused Peter Reynolds to fall to his death, had already injured another employee in the past. Nigel Crockford, who has worked at Cemex in Lawford Road, Rugby, for 14 years, received burns to his eyes during an explosion in 2006.

A company from East Yorkshire has been prosecuted after a worker was run over by a digger and seriously injured. Both Kendra brothers pleaded guilty to breaching section 2(1) of the Health and Safety at Work etc Act 1974.
