Guardrail failure caused new starter to fall.

A waste management firm has been fined £12,000 after a guardrail gave way resulting in a worker falling nearly three metres on his second day at work. The court heard that the incident took place at AB Waste Management Ltd’s facility in Darlaston, West Midlands on 16 June 2009. The worker was cleaning a crushing machine when the railing gave way. He fell to the factory floor, narrowly avoided falling into the machine, and broke his spine in two places.

No survivors found after West Virginia mine disaster

An agonizing four-day wait came to a tragic end early Saturday morning when rescue workers failed to find any survivors in a mine after an explosion earlier this week.
